Addaleigh "Addie" Amelia Huzyak, 30, of Beaver, passed away unexpectedly Monday, June 6, 2016, at her home.
Born April 4, 1986, in Brighton Twp., she was the daughter of Rick (Jan Wagner) Huzyak, Rochester, and Joan (Timothy Rose) Small, Beaver. She was a graduate of Beaver High School, received her B.S. from Youngstown State in Human Health Services and associate degree in criminal justice from C.C.B.C. She was working at LTSR Brighton Rehabilitation and Wellness Center at Friendship Ridge. Addie had also worked at the Butler Crisis Center and the Community Corrections Association in Youngstown. She had also been a waitress at Eat'n Park, Center Twp., Bert's Wooden Indian and started her work history at Wendy's.
Surviving, in addition to her parents, are her brother, Jeb Huzyak, Rochester; two stepbrothers, Nathan Jones and Timothy C. Rose, both of Beaver Falls; maternal grandmother, Shirley Small, Chippewa Twp.; paternal grandmother, Amelia "Millie" Huzyak, Beaver; uncles, Jeff (Lorraine) Small, Beaver, and Ed (Peggy) Huzyak, Murrieta, CA; aunts, Julie Lendick, Darlington, and Pat (Barry) Lansberry, West Mayfield; two special cousins, Jennifer Greenly and her son Gage and Mackenzie Lendick-Fosnot and her daughter Bailee; numerous more cousins and many friends.
She was preceded in death by her maternal grandfather, Joseph J. Small; paternal grandfather, Edward J. Huzyak, and her cousin, Mark Greenly.
Friends will be received Friday from 5 p.m. until time of funeral service at 7 p.m. in the J&J SPRATT FUNERAL HOME, 1612 Third Ave., New Brighton, www.jjsprattfh.com, with the Rev. Jennifer Claxon officiating.
Cremation will take place after the service.
